...where I'm from, I was in the military up until recently. There was an incident where a scout didn't properly describe the area he had scouted beyond answering 'yes' to if there were trolls in the area. Because of that, the team sent to clean out the trolls was unprepared and four people died.
It was eventually ruled to be 'human error', and the scouts were trained to be more thorough in their reports. But nobody was punished for it, not even the scout who made the mistake.
